School Group - Student Nurse Clinicals

A school representative that wishes to bring a group of students, can fill out a form inquiring about the availability of student nurse clinicals. This will connect you to the Clinical Nurse Educator to find out detailed information about NH+C's clinical rotation program, availability, and next steps in the process. 

Due to the limited number of spaces in our program, we cannot guarantee a spot for every applicant or group of students. 
 

Deadlines

To be considered for approval of a student program, requests must be received by:

  • April 1 for fall semester/winter quarter
  • September 1 for spring semester/summer quarter
 
Applicants will be notified of approval/denial no later than:
  • May 1 for fall semester/winter quarter
  • October 1 for spring semester/summer quarter
 

Student Representative Requirements

In order to be considered for a clinical rotation, you must complete the following form and upload all documentation. Please have all documents ready to upload before starting the form.

The form will NOT be submitted unless you have completed all fields.

Required Documents

  • Instructor MN RN Licensure (not required if applying for capstone students)
  • Proof of liability insurance
  • Evidence of program accreditation

Accepted file formats - .bmp, .doc, .docx, .gif, .jpeg, .pdf, .png, .psd, .tif. Files cannot be larger than 25 Mb per upload.

  7. MN Background Clearance Letter
  8. Proof of Personal Liability Insurance

Immunizations Required for Clinical Rotation Positions

Measles, mumps, and rubella (MMR)2 doses of MMR - OR - positive lab titers showing immunity for mumps, measles, and rubella
Tuberculosis (TB)Negative 2 step TST test  - OR - negative blood assay M. tuberculosis (BAMT) test in their medical history with annual negative  TST or BAMT.Observers with a positive TST test must have a negative chest x-ray at least 12 months8 after conversion and negative symptoms.
Hepatitis BCompleted 3 shot series, or positive antibody titer - OR - 2 of the 3 shots completed with series in progress - OR - an observer may document their decision to decline to be vaccinated against Hepatitis B

Does a school representative need to be on site with the students?

Yes, all clinicals require an instructor to be present on hospital grounds EXCEPT a senior student completing their final preceptorship/capstone education.
