Health problems can complicate your life in many ways. Medical social workers are professionally trained to help you with your unique needs during this challenging time. The services of medical social workers are available at no charge to patients – and their families - of the hospital, clinics and outpatient settings.
Elder & Disability Resources
A well-curated collection of the most comprehensive, user-friendly sites, especially those that provide personalized help to match appropriate services to your specific needs.
Support Services
To provide a level of counseling, education, and other support services to both patients and their families/caregivers, or to refer you to others for individualized services. Includes:
- Discharge planning assistance, to help our patients and families plan for a safe discharge from the hospital
- Assistance with healthcare directives
- Coping with diagnosis/treatment
- Caregiving concerns
- Parenting challenges
- Relationship challenges
- Concerns about abuse, neglect or violence
- Financial concerns
Referral to Area Services
To provide information on and/or facilitate referrals for:
- In-home health care to meet nursing and rehabilitation needs
- In-home attendant care for personal care needs or homemaker services
- Rental or purchase of medical equipment
- Area care facilities (assisted living, skilled nursing facilities, short-term rehabilitation, memory care, and respite options)
- End-of-life care options

To put you in touch with available community services, including but not limited to:
- Transportation
- Counseling/Mental health agencies
- Chemical Health and substance abuse treatment programs
- Support groups for specific needs/topics
- Exercise/Yoga/Alternative therapies
