Clinical Rotations Details and Application

To be considered for approval of a student program, requests must be received by April 1 for fall semester/winter quarter and September 1 for spring semester/summer quarter. Applicants will be notified of approval/denial no later than May 1 for fall semester/winter quarter and no later than October 1 for spring semester/summer quarter.

A clinical rotation is a necessary requirement of a student’s academic program. Students are placed in a supervised learning environment in which they put skills developed in the classroom into practice. This application is for individuals who are interested in Northfield Hospital + Clinics Clinical Rotation Program. Due to the limited number of spaces in our program, we cannot guarantee a spot for every applicant.

Required Documentation

In order to be considered for a clinical rotation, you must complete the following forms and upload all documentation. Please have all documents ready to upload before starting the form. 

The form will NOT submit unless you have completed all fields and all forms have been uploaded.

Immunizations Required for Clinical Rotation Positions

Measles, mumps, and rubella (MMR)2 doses of MMR - OR - positive lab titers showing immunity for mumps, measles, and rubella 
Tuberculosis (TB)Negative 2 step TST test  - OR - negative blood assay M. tuberculosis (BAMT) test in their medical history with annual negative  TST or BAMT.Observers with a positive TST test must have a negative chest x-ray at least 12 months8 after conversion and negative symptoms.
Hepatitis BCompleted 3 shot series, or positive antibody titer - OR - 2 of the 3 shots completed with series in progress - OR - an observer may document their decision to decline to be vaccinated against Hepatitis B
